Because physical well-being is essential to both athletic and academic success, the Department of Athletics feels a strong responsibility to help Seawolf student-athletes maintain optimal health.

The UAA Sports Medicine Office asks ALL Seawolf student-athletes to download, print, and complete the UAA Sports Medicine Medical Forms posted below. Please note that there are two sets of medical forms, one set for incoming freshmen and transfer student-athletes (Group A), and one set for returning student-athletes (Group B).  Be careful to download the appropriate set for your situation.  The first page of the download is a Sports Medicine Form Medical Instruction Letter.  Please read the instruction letter in its entirety before completing any forms.  Contact Head Athletic Trainer Chris Volk at cavolk@uaa.alaska.edu for any questions.
